Change of name advertisements are basically published in the newspaper when there is a mismatch in the spelling of someone's name and they do not match with the spelling which is there in their documents. To get the name corrected, a legal procedure is followed by publically announcing in the form of a newspaper advertisement.

Scan copy of complete notarized affidavit required & it should not be more than 6 months old.(The ad matter should be as per the affidavit. Eg: Name, Father’s Name, Surname, Short and Full name etc. to avoid editorial rejection). To get an affidavit from the notary, drafting is done by providing the details of the old name and new name. This is executed on a Stamp Paper of Denomination of Rs. 10/- or as per the prescribed stamp duty as per your state. Then the affidavit is attested and sealed by the Notary Public.

Change of Name ads have a particular format to follow where every details need to be mentioned to avoid rejection from the publication house. Multiple person's Name Change in single ad text is not allowed.
I, {Old Name} S/O or D/O {Father's Name} born on {Date of Birth} residing at {Full Address}, have changed my name to {New Name} vide affidavit dated {Affidavit date} at {Place name}

1. Clearly mention old name and new name in the ad matter.
2. Mention the affidavit no in the ad matter.
3. If the person who's name is being changed is a minor, then his/ her guardian should make the name change declaration.
4. Name of the notary can also be included in the matter.
5. You can also include a residential address within the ad matter whereas its mandatory for some publication.
